Within this guidelines the SafeCare onsite clinics will be customized to meet the needs of the specific organisation based on the healthcare strategies and epidemiological study. These clinics can be easily plugged into any community setting like schools or corporates.

Preventive care has enough challenges on its own; age, presenting illness, family concerns, and then on top of these challenges doctors face on generating complex charts every day in EMR's not designed for the preventive care. The challenge of keeping up and providing quality preventive care is daunting. As the patient volume for preventive care is huge delivering consistent and reliable preventive care through the current primary care model is a formidable task.

The onsite Safecare clinics offers two broad services - Concierge health care services and Preventive health care services

Concierge Health Services
  •    1. Primary care - Examine patients, make diagnosis and provide basic medication. flu shots, vaccination and other shots like Insulin and other illness based medication.
  •    2. Record maintenance - Registration, questionnaire, taking medical histories, assessment, lab report, coordination based on conditions identified and writing case summaries for the subscribed members.
  •    3. Diagnostics - Order and interpret laboratory tests, X-rays, ECGs, Ultra sonogram, Echocardiogram, Treadmill tests, pulmonary function tests etc.,
  •    4. Pharmacy - Dispensing of OTC medicine and organising prescription drugs.

Preventive Care Services

The preventive care services are a preplanned exercise which happens periodically. It follows the SafeCare preventive care model of Screening, Intervention and continuity of care. The system drives the process of preventive care

  •    1. Screening - Periodic biometric and health check up plans for health risks identification, condition management and holistic wellness. All health assessments results are digitalised to provide dashboards to the individuals so as to gain an understanding on the health risk profile and tits rends. Diagnostic AI scans through the assessment reports and past health data and trigger personal health condition risk and build organisation health index in real time. Depending on the organisation's preference either a preventive health check up with blood tests or biometric assessment is performed at a frequency defined by the organisation.
    •       • Preventive health checkups - SafeCare clinic has the capability to deliver on site preventive health check-ups in association with the empanelled hospitals and labs. If the organisation has existing vendors we work with them to digitalise the assessment reports.
    •       • Biometric assessments - Digital Health stations to periodically asses key metabolic risks that has a direct link that are risk factors for NCD - BP, sugar, body weight, SPo2, heart rate and body temperature.
  •    2. Intervention - Based on the screening personalised intervention for every individual is worked out.
    •       •   Onsite intervention - Onsite or Virtual consultation is provided by our GPs or specialists depending on the need of the particular condition and risks of the patient.

Continuity of Care

The screened individuals are classified into high risk, moderate risk and no risk category. The onsite clinic, APP and health coaches provide the education, lifestyle behaviour change support and coping strategies including coaching for exercise, diet and preventive health to help the individuals adhere to the plan provided by the specialists and to self-manage their health condition and wellness.
